Job Overview
The Clinical Lead - Transitional Care plays a pivotal role in coordinating healthcare services and ensuring seamless transitions for patients moving between different levels of care. Responsibilities
- Develop and implement comprehensive transitional care programs.
- Coordinate patient transfers between various healthcare settings.
- Collaborate with & Lead multidisciplinary teams to create individualised care plans.
- Monitor patient outcomes and adjust care plans as necessary.
- Educate patients and their families about care transitions and expectations.
- Ensure compliance with healthcare regulations and standards.
- Conduct staff training and development related to transitional care.
- Analyse data to identify areas for improvement in transitional care processes.
- Facilitate communication between healthcare providers and patients.
Qualifications & Skills
- Degree or higher in Physiotherapy, Occupational Therapy or Nursing or related field.
- Extensive experience in transitional or case management roles.
- Strong background in patient education and advocacy.
- Proven leadership experience within a healthcare setting.
- Familiarity with Electronic Health Records systems.
- Experience in quality improvement and data analysis.
- Minimum of 5 years of clinical experience.
- Excellent communication and interpersonal skills.
- Strong problem-solving and critical thinking abilities.
- Ability to work collaboratively with a multidisciplinary team.
- Knowledge of healthcare regulations and compliance standards.
